New Patient Form
We are registering new Private patients immediately. We have limited funding for new NHS patients. Those patients we assess to have good oral health we will register as an NHS patient as spaces become available. Adults who are in pain or are assessed to need treatment will be examined privately (approx. £89) and will need to undergo a course of treatment (see price list). Patients who have completed a course of treatment are then guaranteed an NHS registration for their future dental care.
Please note NHS places are limited and if you are in need of treatment you will need to be seen privately first
If multiple people from the same household wish to join, please fill in a separate form for each person (and appointment request form)
